1. A Continue CareRx representative will call you to discuss your health status and medications and answer any questions about Continue CareRx.
2. Enrollment can be completed over the phone.
3. You sign a Continue CareRx program consent form.
4. Your medication list is transferred to
PharMerica automatically.
5. A PharMerica pharmacist reviews your medications to
make sure they’re safe and effective.
6. PharMerica synchronizes a monthly cycle for all
your medications.
7. Your first medication delivery arrives in a Continue CareRx
shipping box before your current supply runs out.
8. A nurse conducts regular follow up.
